Print out and send with your cards.

To print the AGS submission form:

Double click the button below and when the order form appears, click the printer icon in the upper left-hand corner of the order form. Note: Do not click the print button of your browser as the order form will not print out.

If you would like the order form e-mailed directly to you, please send us an e-mail with the subject line "AGS Order Form." This will allow you to have the order form on your computer and you will be able to access it without going on-line.

You will need Acrobat Reader, version 3.0 or higher, to be able to read the PDF. You can download the latest version of Acrobat Reader by double-clicking the icon at the upper-right hand corner of this page.
